We report the case of a 51-year-old female patient with severe chronic heat urticaria (HU), a rare form of chronic inducible urticaria (CIndU). Skin symptoms started at the age of 49 years, when our patient first observed that direct skin contact with hot water while showering/bathing, to hot steam, hot objects, higher room temperatures or exposure to hot sun radiation resulted in flat erythematous wheals and angioedema. Heat-induced signs and symptoms usually lasted for 30 min up to 2 h and were very itchy and even painful with burning sensations. Our patient’s quality of life (QoL) was substantially impaired. We could exclude further CIndUs, as she did not report any complaints associated to cold water, exercise or sun radiation during the cooler months. There was no co-occurrence of chronic spontaneous urticaria.
